The correlation between liver iron concentration in hemochromatosis patients and multiple scoring systems in King Abdulaziz Medical City, Saudi Arabia

Abstract
Objective:
This study aimed to assess the correlation between liver iron concentration (LIC) and multiple scoring systems, including fibrosis 4 (FIB-4) and model for end-stage liver disease (MELD) scores, in hereditary and acquired hemochromatosis patients at King Abdulaziz Medical City (KAMC) in Riyadh, Saudi Arabia.
Methods:
A cross-sectional retrospective study was conducted on 267 patients with recorded LIC. Patients without LIC or necessary scores (FIB-4 and MELD) were excluded. Data on age, gender, body mass index (BMI), aspartate aminotransferase (AST), alanine transaminase (ALT), alkaline phosphatase (ALP), ferritin, sodium, creatinine, and total bilirubin were analyzed. MELD and FIB-4 scores were calculated using online tools.
Results:
The majority of patients were male (59.9%), aged under 35 years (55.4%), and had acquired hemochromatosis (69.8%). Higher AST, ALT, ALP, and ferritin levels were significantly associated with abnormal LIC (p<0.05). Younger age (p<0.001), female sex (p<0.001), lower BMI (p<0.001), acquired hemochromatosis (p<0.001), sickle cell disease (SCD) (p<0.001), thalassemia beta major (p-value=0.006), and high MELD score (p-value=0.037) were significantly linked to abnormal LIC. Multivariate regression analysis identified younger age (AOR = 0.336, p<0.001), female sex (AOR = 3.115, p<0.001), SCD (AOR = 2.971, p-value=0.001), and thalassemia beta major (AOR = 3.126, p-value=0.012) as independent predictors of abnormal LIC.
Conclusion:
Abnormal LIC was more prevalent in younger patients, females, and those with acquired hemochromatosis and SCD, highlighting the need for regular monitoring for early intervention. Further studies of larger sample sizes are recommended.
