A systematic review and meta-analysis comparing the effects of labetalol and hydralazine for hypertension on fetal and neonatal outcomes in pre-eclampsia

Abstract
There is a lack of comparative data on the effects of hydralazine and labetalol on fetal and neonatal outcomes in pre-eclamptic patients. This systematic review and meta-analysis aimed to compare their impacts on fetal heart rate (FHR) abnormalities, 1-minute appearance, pulse, grimace, activity, and respiration (APGAR) scores <7, neonatal intensive care unit (NICU) admissions, and neonatal death in pregnancies complicated by hypertension due to preeclampsia. A thorough search was conducted across three databases up to the year 2024. The quality of included randomized controlled trials was evaluated using the Cochrane Risk of Bias 2 (RoB) tool. A random-effects meta-analysis was performed with p-value<0.05 considered statistically significant. In the eight selected randomized controlled trials, five studies had a high risk of bias, two had a low risk, and one had some concerns. No significant differences were identified between hydralazine and labetalol in FHR abnormalities (OR 0.350; 95% CI: 0.056–2.179), 1-minute APGAR scores <7 (OR 1.018; 95% CI: 0.647–1.603), NICU admissions (OR 0.920; 95% CI: 0.632–1.338), or neonatal death (OR 0.748; 95% CI: 0.220–2.538). This systematic review and meta-analysis found no significant differences in fetal and neonatal outcomes between hydralazine and labetalol. These results align with current clinical guidelines and emphasize the need for further research into long-term outcomes and patient-centered treatment strategies.
