Risk factors of post-ureteroscopy sepsis: a systematic review and meta-analysis

Abstract
With the expansion of Ureteroscopy (URS), infectious complications have become problems that can't be ignored, and sometimes lethal morbidities can occur. The present study aims to determine the risk factors of infectious complications (sepsis) post-URS patients. A systematic search was performed over different medical databases to identify Urology studies, which studied the outcome of the Sepsis URS group versus the Control URS group of post-ureteroscopy patients. Using the meta-analysis process, either with fixed or random-effects models, we conducted a meta-analysis on risk factors (gender, diabetes mellitus, and stent insertion) as primary main outcomes. Six studies were identified involving 3660 patients, 169 in the Sepsis URS group, and 3491 in the Control URS group. The meta-analysis process revealed that the overall pooled prevalence of sepsis in URS patients = 6.3 %. We found a highly significant increase in sepsis in female URS patients and a significant increase in prior stent insertion, compared to the Control URS group (p < 0.001, p = 0.034 respectively). But we found a non-significant difference in Diabetes Mellitus (DM) prevalence in the Sepsis URS group compared to the Control URS group (p > 0.05). To conclude, patients who undergo ureteroscopy after ureteric stent insertion have a higher risk of postoperative sepsis. Prolonged stent dwelling time, sepsis as an indication for stent insertion, and female gender are independent risk factors. Stent placement should be considered cautiously, and if inserted, ureteroscopy should be performed within 1 month.
