Fasting for lipid profile in clinical practice, do we need to ask the patient to fast?

Abstract
Background: Fasting has always been required to assess lipid profile which is not practicable for all patient groups. Recent studies have stated the effectiveness of non-fasting lipid profiles. In the present study, we aimed at evaluating whether non-fasting lipid profiling is really recommendable as stated by previous studies. This study could change the policy of lipid profile testing which could better facilitate and increase patients compliance toward it.
Methodology: A prospective study was conducted involving patients aged between 18 and 90 years of both genders attending an outpatient clinic at Imam Medical Center in Imam Mohammad Ibn Saud Islamic University in Riyadh. Data were collected by withdrawing blood samples in the non-fasting state and comparing the findings to previous fasting lipid profiles obtained between September 2020 and May 2021. Data analysis was performed using Microsoft Excel and Statistical Package for the Social Sciences software.
Results: Statistical analysis using paired t-test showed that the average total cholesterol, low-density lipoprotein (LDL), and high-density lipoprotein (HDL) levels were not significantly different between the fasting and non-fasting states (p = 0.621, 0.21, and 0.39, respectively). However, the average triglycerides levels were significantly different between the fasting and non-fasting states (p = 0.03).
Conclusion: Our results indicate that there is no appreciable difference in the average levels of LDL, total cholesterol, and HDL between non-fasting and fasting lipid profiles. Up until now, there is no strong scientific evidence to support.
