Abstract
Background: Colorectal cancer (CRC) is the most common cancer in men and the third most common cancer in women in Saudi Arabia. Carcinoembryonic antigen (CEA) is a complex glycoprotein released by various malignancies, including CRC. It is present in the blood at a modest concentration. The present study aimed at measuring the relationship between CEA levels and colon cancer stage in patients treated at the King Abdulaziz University Hospital (KAUH).
Methods: We conducted this retrospective record review study in the Surgery Department at KAUH, a tertiary facility in Jeddah, Saudi Arabia; the study included 285 patients diagnosed with CRC between 2015 and 2021.
Results: There was a significant relationship between preoperative CEA level and CRC staging (p = <0.05). Also, we found that the mean postoperative CEA was significantly higher in patients with metastasis compared to non-metastatic patients (184.7 ± 600.79 vs. 4.08 ± 10.95).
Conclusion: Our results showed a relationship between preoperative CEA level and CRC staging. Additionally, postoperative CEA level was associated with metastasis. We recommend a clinical trial to follow up and measure CEA multiple times.

Key words: Colorectal cancer , carcinoembryonic antigen, general surgery, oncology
